Waiver, Release, and Assumption of Risk Agreement

Important Notice: This document serves as a vital legal agreement outlining the assumption of risk associated with participating in services, workshops ,and offerings provided by Alchemist Path LLC, One Center, and Roxanne Story.  By engaging with any services--whether in person, online, or through affiliated products--you acknowledge and agree to the terms of this Waiver, Release, and Assumption or Risk Agreement.  Please read carefully before proceeding.

 I, (Participant Name), voluntarily participate in services, workshops, programs, or appointments provided by Alchemist Path Wellness and/or Roxanne Story, including but not limited to:

·  Massage therapy, bodywork, and related techniques

·  Quantum Healing Hypnosis Technique® (QHHT®) and related modalities

·  Coaching, consulting, intuitive services, and mindset training

·  Energy healing, Reiki, sound healing, tuning forks, and vibrational therapies

·  Facials and related techniques

·  Use of essential oils, skin care products, supplements, and holistic products/services

·  Any other current or future products or services offered

I understand and acknowledge that these products and/or services are not a substitute for medical, psychological, or legal advice.  No guarantees or warranties are made regarding specific outcomes.  I assume full responsibility for my health, choices, and actions during and after participation.

 In consideration of receiving services from Alchemist Path Wellness and/or Roxanne Story, , I agree to:

·  Assume all risks related to participation, including potential allergic reactions, discomfort, or unintended effects from treatments, products, or techniques.

·  Release Alchemist Path Wellness, and/or Roxanne Story, employees, contractors, affiliates, and associated individuals from any claims, damages, or liabilities, including those arising from negligence.

·  Acknowledge that participation in services, workshops, and programs is voluntary and at my own risk.

I acknowledge that Alchemist Path Wellness, and/or Roxanne Story employees, affiliates, and associated individuals do not provide medical, psychological, or therapeutic diagnosis or treatment.  Any advice or guidance provided is for informational and personal growth purposes only.  It is my responsibility to seek appropriate professional medical or mental health care when necessary.

 I understand that Alchemist Path Wellness and/or Roxanne Story may promote, recommend, or be affiliated with third-party products and services.  Any affiliate links will be disclosed as required by law.  I acknowledge that purchases made through these links may result in commissions earned by the business.

  By signing below or continuing to use Alchemist Path Wellness services (in person, online, or otherwise), I confirm that:

·  I have read and understood this agreement.

·  I waive my rights to bring legal action against Alchemist Path Wellness, Roxanne Story, employees, affiliates, or associates for any reason, including negligence.

·  I understand that policies and terms may be updated periodically, and my continued participation signifies agreement to any changes.

By continuing to use this website, participate in services, or engage with any offerings from Alchemist Path Wellness and/or Roxanne Story, employees, affiliates, or associates you acknowledge and agree to the terms outlined in this Waiver, Release , and Assumption of Risk Agreement.

Parent or Guardian is required  to be present for in person massage/facials-if under the age of 18 years.  Signature required by Parent or Guardian
